NOTE: Required forms and documentation must be completed and returned to the City of Surrey Human Resources Department prior to your first day of employment.

1. Review all Policies and Practices

2. Complete & Submit Forms to Human Resources
Forms to send to City of Surrey Human Resources:
- Acceptance of City of Surrey and Surrey Fire Service Policies & Practices and Employment Offer Form
- Personal Information Form
- Proof of SIN Form
- Payroll Direct Deposit Form
Optional Forms
The City of Surrey will deduct the standard income tax from your earnings. If you want more or less income tax taken, please complete, sign and return the following TD1 tax forms:
Please return all required forms and documentation to City of Surrey Human Resources by:
- Uploading them using our secure system through the job offer link that we emailed to you; or,
- Faxing them to 604-591-4517; or,
- Mailing or physically returning them to City of Surrey HR, 13450-104th Ave., Surrey BC V3T 1V8.
The City of Surrey recommends that you do not email the required personal information using unsecured systems.
NOTE: You may not start work without the paperwork being received and accepted by Human Resources.
