Backflow Preventer Forms and Procedures
Find all the forms necessary for reporting backflow preventer tests, removals and replacements.
Existing Backflow Preventers
For all existing backflow preventer devices, online test report submission is mandatory. A paper copy of the test result is no longer accepted by the City. This method is taken to improve the efficiency of the process:
- The consumer or the tester will no longer be required to get a blank test form and subsequently submit a completed test form to the City; and
- The City will reduce the process of manually entering test reports to the database.
New or Unregistered Backflow Preventers
For newly installed or previously unregistered backflow preventers, a paper copy of the test report must be filled in and sent to the City. Clearly legible PDFs can be submitted to email@example.com.

Plumbing permits are required for all new or previously unregistered backflow preventers.
All fees including test report fees are due at the time of your permit application.
The tester must submit the test report within 14 days of the date of the test to firstname.lastname@example.org. There has been an update to the Test report form that includes a space for the Plumbing permit number. If no valid permit number is provided, you will need to get one before the reports are accepted.
Replacing a Backflow Preventer
If any part of an existing backflow preventer device is replaced, the new/replacement device will be tested immediately following installation. The test report needs to be submitted online through BSI Online, including the information on the device being replaced.
Removing a Backflow Preventer
A Removed Backflow Preventer Verification Form must be completed by the consumer to confirm that the backflow preventer, and the hazards it isolates, have been removed. After the City receives your form, we will de-activate the backflow preventer. We won't issue further notice for testing of the device.
